On the 15th of September 1940 this aircraft lost height on take off and crashed soon after at 15.30hrs. The aircraft burnt out, Cat W/FA damage being sustained. The survivors were severely burned but were rescued.

The aircraft was built to contract 791587/38 by Lockheed at Burbank, California. After delivery to the UK and acceptance it was delivered to 220 Squadron in September 1939.
